Please join us for our NAFA Midwest Chapter Golf Outing 2021, taking place at the Otter Creek Golf Course

Ankeny, IA 50021
Cost: $25.00 per person.

Time: 10:00 am Shotgun Start.
Registration and breakfast will begin at 8:45 am.

•   18 Holes
•   Golf Cart
•   Breakfast

•   Drink Ticket
•   Awards

Please register prior to September 1 so final arrangements can be made with the course. If you want to golf in a certain foursome, please email your requests to jgarrison@fmh.com. 

Every player must have golf clubs. If you need to rent clubs, please contact Otter Creek Golf Course directly at 515-965-6464

There are 17-hole sponsorships available. If you would like to sponsor a hole, please click on the link below to register.

Prizes are also welcome.  If you would like to donate any items for prizes, please email Julie Garrison @ jgarrison@fmh.com with the specifics on what you will be donating.
